Noble Care LLC provides community-based treatment that promotes self-determination, empowerment, recovery, resilience,
health and the highest possible level of individual participation.

Opportunities are available for private rooms.

Services

Daily nursing and rehabilitative care will be performed only by, or under the supervision of, skilled medical personnel which is our Nurse Facilitator. This care will be given to those who need care that has orders from a physician and must follow a plan of care.

Candie’s Place

At Noble Care we encourage creativity and freedom of choice. Individuals will experience joy and satisfaction through daily living experiences that will enhance quality of life, develop personal skills and community integration through teaching and volunteering.

We provide training and support to strengthen community-based living in a family-like home setting. We provide skills training in the environment where the individual will live, recreate, and actually use these skills.

• Individuals and family will be included in developing individualized support plans.
• Link individuals to support groups, vocational, and educational opportunities in recreation.
• Assist individuals in developing positive self-images and overcoming stigmas.
